Medication Reconciliation - Surgery/Procedure

Hi!

I’m reaching out to gather insight into your practices regarding the handling of inpatient medications around the time of surgery/procedure. I'd appreciate your input on the following:

1) Do you typically hold or suspend medications perioperatively? If so, for how long?
2) Are any medications discontinued as part of your perioperative workflows?
If yes, is there a streamlined or standardized process within your EMR to reorder or resume them postoperatively?
3) Do you perform a transfer-of-care medication reconciliation (e.g., from floor to OR/PACU and back) as part of your process?

Any details you can share about workflows, clinical decision support, or EMR tools used would be greatly appreciated as we review and potentially optimize our own processes.
