Our electronic health record (EHR) currently requires patient reverification (patient armband scanning) on the medication administration record (MAR) after 10 minutes. Our EHR does not provide any recommendations regarding this setting, but 10 minutes is the default if a value is not provided.

We use single sign-on (SSO) with our EHR. If a practitioner moves from one computer to the next with SSO, the previous EHR session is pulled to the new computer. We have identified a risk with medication administration to the wrong patient with the combination of SSO pulling the previous EHR session and the patient reverification on the MAR set to 10 minutes.

Example, a nurse follows appropriate barcode medication administration (BCMA) workflow for patient A. The nurse then goes to patient B, uses SSO, and inadvertently uses patient A's MAR for medication administration as 10 minutes had not elapsed between patients (the nurse was not prompted for patient reverification).

Additionally, the patient reverification on the MAR setting could also pose a risk with a practitioner using the same computer between patients, like a computer on wheels.
