Methotrexate Education

Hello,

I am reaching out to see what other institutions are doing with methotrexate patient/family education and what they have implemented to meet ISMP's Best Practice # 2. How do you handle patient and family methotrexate education? How do you handle readmissions? If a patient is readmitted wihtin 90 days, 6 months or 1 year do you re-educate? Would be interested in any SOPs you have to share.
