Vancomycin Dilution and Administration Rates

Hello,
Our health system is currently reviewing adult vancomycin order builds. While the package insert advises that vancomycin IVPB concentration remain /= 1 g?
3) IF you use concentrations > 5 mg/mL?
- What dose(s)/volume(s)/duration(s) is/are used?
- Is central line preferred or required?
- Any issues with patients tolerating infusions?
