How does BUD compare to spike by time?

Andre Tran's picture

Forums:

Hi all,

There was some discussion in our pharmacy regarding BUD vs spike time. Let's say we have vancomycin 750 mg IVPB that was compounded with a BUD of 8/13 @0600. If a nurse were to hang it at 8/13 @0559 is that still okay knowing it is an hour infusion? Or

How do your facilities and USP interpret that? If the above scenario holds true, how does one interpret longer infusions (>4h)...nobody goes back and thinks "well it's past the BUD, but what time was it spiked?"

Any feedback is appreciated! Thanks.

Incorrect IV Diluent Volume Errors

Hello!
Looking for ideas/suggestions to avoid incorrect compounded IV preparation errors, specifically selection of wrong diluent bag volume. We use the DoseEdge IV workflow system with barcode scanning enabled, however it only triggers an alert if the wrong product is selected and not based on volume.

A) For those who use DoseEdge, have you experienced similar issues? what hard stop mechanisms do you employ to prevent inadvertent preparation using the incorrect volume?

Oral Chemotherapy Guideline or P&P

Forums:

UCHealth is interested in creating a guideline or policy and procedure for the safe prescribing, dispensing, administration and monitoring of oral chemotherapy agents. If anyone has something similar and is willing to share, we would be very appreciative.

Event type classification

Forums:

For those who receive error reports from both inpatient and outpatient/retail settings, and/or working with State BOPs or PSOs on error report databases, what classification do you use for event TYPE?

The commonly used classifications seem to be tailored to certain practice settings but we are looking for a single solution that would capture event types from all settings, wondering what others are doing and share pros/cons to your method.

Hey all,

I have another quick question that I wanted to run by the group. For those who have Myxredlin on their formulary, how do you handle bags that have been removed from the carton and refrigerator but may not have been used? How are you working with nursing to prevent waste?

Thanks in advance!
Jameika

Amber Oral Syringes

Forums:

Good afternoon group! This may be more applicable to you who have a significant pediatric population, but I'm interested in everyone's thoughts.

How many of you are using amber syringes for your oral doses at your facility?

Do you require their use only for repacked items that are going to be stored for a period of time, or for all doses dispensed?

What feedback have you received regarding ability to determine product color or detect air bubbles?

Thanks,

Kevin

Controlled Substance Documentation Time Allowance

Forums:

Hello Colleagues,

We just got cited for something so hoping I can get some benchmark data from all of you.

Does anyone have in policy, the time allowed for documentation on the MAR and waste in the ADM of a controlled substance once removed from the automated dispensing cabinet.

I've heard some folks allow nursing up to an hour to get their documentation and waste completed once they remove a CS from the automated dispensing machine.
