Infuse over time

I just joined a new institution and a lot of the intermittent medications do not require an "infuse over" time as part of the order (even antibiotics). The nurse looks up what is allowed and uses that as a guide.

I was under the impression (possibly wrong) that this was required. Does anyone have any literature, opinions or descriptions of how you operate?

Provigil contraindication from Health Canada

Hello!

Wondering if anyone else's organization was doing anything with the heightened warnings / contraindication that Health Canada released in June 2019 regarding the risks of use of Provigil during pregnancy. (Currently, in the US, the FDA has not released any updated stance on the matter as Canada has)

Sodium or Potassium Phosphate Rate of Administration

At your institution, what is the protocol regarding rate of administration of IV sodium or potassium phosphate? Please answer considering a dose of 15 mmol.
a) Max 10 mmol/hr regardless of dose
b) 6 hour infusion time regardless of dose
c) Other- please specify dose and rate of administration

Maximum Tablets to Make a Dose

Hi All,

We have had a few errors related to the number of tablets to create a dose. We decided to take a closer look at this, since there was also an error published with 38 tablets of Bactrim. We have anecdotally discussed using 5 as a cut off. We are currently operating on AllScripts SCM and transitioning to Epic in 2021.

1. Does your institution have a maximum number of tablets to create a dose? If yes, how many?
2. What evidence did you use to determine this number?

Removing overwrap from IV premix bags

Hello,

Wondering what policy you have in place for removing overwraps from IV premix products? We had certain dating in place based on the size of the bag, but are running into different products now stating "use promptly" once removed from the overwrap. We are instituting a policy of not removing any overwraps so that we can try and cut down on the multiple dating schemes we have.

Nebulized TXA

Is anyone administering nebulized TXA at their facility and have you had problems with fogging? An ED provider ordered it for hemoptysis in my facility and it created so much fog in the patient's room that the fire alarms went off. Is there preferred equipment or methods Respiratory Therapy may be able to provide that could decrease this? Thanks.
