Rabies,Varicella and Hepatitis B Immune Globulins Documentation

There is a lot of information out there regarding Federal requirements for documentation of vaccines (lot#, expiration date, Mnfr name, etc), but I am having a hard time finding anything about Immune Globulins. Is documentation of those products similarly regulated by federal agency? What information does your institution documents, upon administration of Varicella, hepatitis, Rabies IGs? Any information/reference is much appreciated.

Anesthesia BCMA Survey

We have been keen on implementing BCMA in anesthesia for over a year, but have been running into resistance from end users.

I wanted to conduct a short survey to gather data to see if it can help us move further.

1. Have you implemented BCMA in anesthesia/procedure area in your institution?

2. If yes, which EMR do you use?

3. Did you face any barriers from end users? If yes, how did you overcome them?

Clear definitions and differentiation of STAT, NOW, One dose orders

we noticed that some prescribes are missus STAT orders or they are miss-understanding the differences between STAT, NOW and one dose.
As result i want to see if you have clear definitions with time frames.

Are sites light protecting TPNs for neonates and children <2 yrs given the recent recommendations from the European Medicines Agency? If yes, do you light protect the IV line in addition to the bag?

Or are some institutions waiting until ASPEN releases their position paper?

Bleomycin and Oxygen clinical decision support

Does your institution have clinical decision support or alerting to help prevent concomitant ordering of supplemental oxygen for patients on or have historically received bleomycin therapy? (increased risk of pulmonary toxicity/pulmonary fibrosis with supplemental oxygen combined with bleomycin)

Hyperkalemia Protocol

Hello Everyone,

We are reviewing our current practice and orderset for the treatment of acute hyperkalemia and I am interested in what other hospitals are doing.
• Has your site/health system removed kayexalate from formulary (Y/N)?
• Has your site/health system replaced kayexalate with Lokelma or added Lokelma
as an alternative?
o If so, what is the dosing regimen used?
• What systems are in place to ensure the safety of insulin administration?

Grace Period for Early Administration of PRN Medications

Does your institution allow a "grace" time window for early administration of PRN medications (e.g. opioids)? Our current policy is that the prescribed time interval must elapse before giving a new PRN dose, but considering a change to allow a "grace" period in light of increasing attention/audits of controlled substance administration.

A) No "grace" period for early administration of PRN medications is allowed - the prescribed frequency time interval must elapse before a new dose can be given.

B) Grace period is allowed - 15 minutes

High alert medications

for high alert medication our policy restricted availability of HAM TO ONLY ICU , PICU,OR and ER but sometimes we need heparin and lidocaine inj. for some ward example surgical ward for catheter maintenance(heparin fush) and as local anethetic (Lidocine) in dressing room what iwil do .iam looking for any policy or plan for this idea
