Epic Low Dose Alerts

We have had some great catches where low dose anti-biotics are ordered for pediatric patients (i.e. 150 mg vs. 150 mg/kg). What Epic solutions have you utilized to monitor this? Hesitant to turn on all low dose warnings across the system due to the potential for alert fatigue.

Proactive Naloxone Order

Hi,
Would love to hear if anyone has a requirement for a naloxone PRN order for specific patient populations or when specific medications are ordered. For example, when insulin is ordered we have a few hypoglycemia orders that must be added to the patient's MAR as PRN. So the question arose, should we do this with opioids & naloxone.

Volatile Chemo Spill Hazard/Risk Assessment

For chemotherapy agents that vaporize (e.g., Carmustine, Cyclophosphamide, Doxorubicin, Fluorouracil), has anyone completed a hazard/risk assessment of exposure once they have been further diluted for administration? Specifically, do you handle a spill of a diluted preparation the same as you would (e.g, require PAPR), if undiluted liquid was spilled?

Methocarbamol associated phlebitis

One of my facilities frequently administers methocarbamol slow IV. Recently, they acquired the generic product from Somerset Therapeutics and have had a few instances of phlebitis, even when diluted and given very slowly. I'm reaching out to see if this is an isolated issue or if other facilities administering slow IV push have noted an uptick in phlebitis. Of note, they have not had this issue in the past with the brand name product.

We’re exploring opportunities to expand our medication safety team and would appreciate insights from other organizations.

For those of you with multiple team members working under the umbrella of medication safety:

-What titles do you use for those roles?

-Do you have more than one Medication Safety Officer?

-Are your team members titled as Medication Safety Pharmacists, Nurses, Coordinators, or something else?

Any details on structure or role differentiation would be incredibly helpful. Thanks in advance for your input!
