Evolution of Anticoagulants and the Effects on Patient Safety

Program Overview:
In the past 5 years, the anticoagulant marketplace has undergone the most significant changes in the history of anticoagulation therapy. These changes, combined with increased use of reversal agents, e.g., prothrombin complex concentrates (PCCs), the optimization of smart infusion pumps, and computerized physician/provider order entry (CPOE) have changed the anticoagulation safety landscape in fundamental ways.
Recent errors and potentially hazardous situations that have been reported to ISMP will be discussed. Dr. Gulseth will provide an overview of the new anticoagulants that have entered the marketplace, focusing on potential risk points where errors may occur. Further, he will discuss the contemporary use of PCCs and the common errors that are seen with their use. The presentation will conclude with a discussion of practical steps that can be taken to modify contemporary medication use systems in order to prevent errors with anticoagulants.

Objectives:
Following completion of the webinar, participants will be able to:
- Describe errors and potentially hazardous situations associated with anticoagulants and prothrombin complex concentrates (PCC)
- Evaluate the current target specific oral anticoagulants with a special focus on error potential
- Compare and contrast the currently available PCCs and their place in therapy
- Outline strategies for preventing anticoagulation and PCC errors using technology and other system changes
