Our organization currently has "team-based" pharmacists that are responsible for admission, transfer, discharge order review, along with all order verification and other clinical activities for the patients on their team. Pharmacy leadership would like to explore adding a role for an individual who could help alleviate some of the strain on the team-based pharmacist so that they can spend more time on patient education, didactic education for learners, and other clinical activities for the patients on their team.
Do any of your organizations use a similar model with both team-based pharmacists and a central order review role? Can you share what the break down of responsibilities is for this model?
