Improvements to Medications for discharge

We had a patient discharged home with correct medication prescription and counseling but could not fill the prescription due to prior auth delays. Has anyone discovered a workflows or epic capability to increase medication safety/medication reconciliation around discharge planning (visibility check off for prior auth, counseling, rx sent to pharmacy, workflow to check discharge orders to outside pharmacies, etc.)?
