Hello - We are currently evaluating the possibility of adding additional anticoagulation pharmacist support at our institution. We are not able to wait for RxOps data, so answering the following questions would be very helpful. Thanks!

1. Do your hospitals or clinics have an anticoagulation pharmacist? If so, how many?
2. Are they providing inpatient or outpatient anticoagulation support?
3. How many patients on average does each pharmacist manage (inpatient/outpatient)?
4. Do the pharmacists bill for their services?

I was wondering if any of you have an effective process for evaluating anticoagulation safety practices and measuring the effectiveness of the actions. We are trying to determine metrics that are accessible without conducting a full MUE.
