Administration of Meds Post Hemodialysis

Hello fellow Med Safety officers!

Hoping to get ideas on how to improve the following workflow.

To facilitate the appropriate administration of scheduled medications (e.g. antibiotics) post dialysis & minimize the risk of a missed dose, our pharmacists currently set MAR times to evening/bedtime hours and add a standardized smart phrase to the admin instructions section of the order to administer the dose as scheduled, except after HD on HD days.

Have any of your organizations leveraged system safeguards to enhance this process and minimize the need for human reliance on the above activities?

If so, what safeguards were implemented? If there were barriers to implementation, what were they and how were they overcome?

Appreciate any and all feedback that you're willing to share.
