Do you require orders for lidocaine 1% & 2% for line starts?

Hi Everyone, Happy Friday!
For over 20 years we have allowed lidocaine 1% and lidocaine 2% (with and without epi) to be stocked in our ADC's and removed without orders. Nobody knows the history of why/how this started but we would like to start requiring orders for these medications.

This will be a BIG change for our institution and there are so many different types of lidocaine that I am not sure where to start.

Does anyone have an order panel to help providers order the correct product (1% vs 2%, with and w/o epi) that you'd be willing to share? Has anyone else also tackled this issue?
