FK Fluid Bags

Has anyone had any reported issues of the blue exit port on FK bags falling off when the nurse spikes the bag causing the fluid to fall out?

We have had 3 reports in the last week on both the NS 100 and the NS 250.

**NS 250 lot is 6402421**
