IV Infusions Running after Discontinue Order

Has anyone come across issues with IV infusions still running on patients after a discontinue order has been placed? If so, what have you done to address it? I'm just starting to look into the workflow, but it sounds like it's mostly manual process without any alarms, alerts, etc., despite Epic/Alaris interoperability being in place.
