Kphos and NaPhos standardized concentration ?

Recently our system clinical team is proposing a standardize concentration for both medications to 15mmol/100mL
As the MSO I am pushing back d/t the LASA and dangers associated with
Potassium Phosphate and Sodium Phosphate mix up

Anyone with any experience in this standardization type where both are the same concentration 15mmol/100mL ?
Any ideas ?
Any errors ?
