Optimizing MAR Instructions for Holding Anticoag

Hello MSOS,

We are looking to optimize the language that we currently have on our MAR around holding anticoagulation prior to invasive procedures/surgery. This is not a one-size fits all plan, and we've changed the wording a few times over the last 5 years.

Does your site have MAR language for holding anticoagultion? If yes, would anyone be willing to share?

Our current statement on the MAR:
Enoxaparin: "Discontinue enoxaparin thromboprophylaxis or therapeutic enoxaparin at least 12 or 24 hour (based on dosing interval), respectively, prior to surgery/invasive procedure, unless otherwise directed by provider for specific procedures."
Heparin: "Discontinue heparin SC at least 8 hours prior to surgery/invasive procedure, unless otherwise directed by provider for specific procedures."
