Clopidogrel and Omeprazole Drug-Drug Interaction

Good Afternoon,

Our organization received a request to re-evaluate drug-drug interaction severity level of the DDI between Clopidogrel and Omeprazole. Currently, the DDI level is a soft alert for providers, however, there is a request to increase the severity to a hard stop which would require override alert. In our organization, assigning the DDI level to a drug pair requires primary literature review. When there is a request to either raise the severity of the DDI drug pair or entering a new DDI from an end user, we require submitting the request with primary literature. However, we recognize this may not be possible for a clinician and the request is submitted using tertriary sources such as LexiComp or Micromedex. Lexicomp classifies this DDI as "Avoid Combination".

How does your hospital system handle this DDI?
