Carrier Fluid and Smart Pump Interoperability

Hello all!

I’d like to hear what your experience is with usage of smart pump interoperability for carrier fluids and if you found ways to successfully accommodate nursing workflow.

A large amount of our non-compliant pump programming cases are related to normal saline for use with IVPBs. There are either zero attempts or failed attempts with ‘order has not been verified by pharmacy’ errors.

ceftriaxone/LR or Calcium incompatibility

We are experiencing increased use of LR as maintenance fluids. How are institutions handling Y-site administration of ceftriaxone with LR? Trissel’s does show one study compatible for 4 hours with ceftriaxone 100 mg/ml y-site administration with LR, but all other studies incompatible.
Do you allow Y-site administration of ceftriaxone with LR?
Do you recommend a flush before and after to mitigate potential precipitation?
If so, what volume is sufficient to flush for compatibility concerns?

Fake Eyelashes in Sterile Compounding Area

We are reviewing our sterile compounding policies and are curious how other hospitals are handling fake eyelashes.

Do you specifically identify and prohibit the use of fake eyelashes in your Sterile Compounding Policies?

If no, do you consider fake eyelashes a "cosmetic" and require that they are removed prior to entering sterile compounding area?

Ambulatory wristbands

Do you currently wristband patients in your Ambulatory clinics?
If yes, do you wristband all patients or certain subsets of patients? (which subsets of patients / how do you determine if patient needs a wristband?)

Shellfish or Iodine/Betadine Allergies and Iodinated Contrast

In reviewing some data on IV contrast I am finding that there is no cross-reactivity for these agents in patients with shellfish or iodine allergies, despite a common belief that there is.

Do you have any policy/procedures at your institution requiring an RN to complete an in person handoff with heparin infusions (current rate, last ptt, last titration, etc) when the patient transfers, much like a controlled substance chain of custody handoff.

Cystatin C-based eGFR for drug dosing

Our nephrology team is encouraging a shift towards Cystatin C-based eGFR for drug dosing. One of our concerns is around practicing "off-label" since most FDA approved dosing is based on CrCl cut-offs. Has this been evaluated at your institution? Please select the options that match your practice, and feel free to expand upon your response in comments:

1) Serum cystatin C (Scys)-based eGFR is used for renal drug adjustment in place of Serum Creatinine (SCr)-based eGFR
