For those with CRP programs and helped build it, such as CANDOR or IHI's PACT Program, how do you recommend bringing this forward to your system leadership? How did you implement or build the case? What were the greatest hurdles needed to overcome or something you wish could have been done differently?

In other words, I'm looking for advice on how to get started, and for experience that we can model ourselves after.

Pharmacist's Role in Releasing orders in EPIC

We are transitioning to EPIC this fall and one of the work flows that has been suggested is that pharmacists will be responsible to release the orders for certain patients such as cath lab and surgery etc.
I am reaching out to see if any facilities follow a similar process. My limited experience with EPIC and releasing orders usually involved the physician or the nurse.
Any input is greatly appreciated

Micromedex Formulary

Do you use the Micromedex formulary platform for maintaining your hospital/health system formulary? We are planning on implementing this product and would like to learn more about how drugs are noted in the formulary that have restrictions or other unique notations. We understand that the Micromedex Formulary feature allows for custom 'tiers' and would love to know what 'tiers' others have developed or any other words of wisdom for effectively implementing this product.

Alaris Software Update v12.1.3

Has anyone reviewed the new update by Alaris? This update from July 15, 2022 incorporates updates to prevent cybersecurity vulnerability but is incorporated with the original update from Feb 4, 2020, which is still not approved by the FDA. Is anyone planning on discussing, or implementing this update? or is anyone planning in purchasing new pumps?

Personal SubQ Insulin Pump Use Inpatient

Hello,

My hospitals do not employ a CDE, nor to they have regular consult assess to an endocrinologist. We are re-evaluating our patient own subcutaneous insulin pump policy, and based on the 2016 ISMP recommendations, I am concerned about our ability to provide the safest care to patients by allowing personal insulin pump use in house without specialist oversight/consultation. Have other organizations successfully supported patient personal insulin pump use without CDE or endocrine specialists assisting in the care of these patients? Insights much appreciated.

Pork-avoidance policy

I wanted to see if anyone had a P&P for assessment on admission for those that avoid medications due to cultural or religious objections to pork or broader cultural assessments on admission. We have a policy but have had issues putting it into action and assuring the right questions are asked before medication administration.

Medications for Fetal Surgery

Hi All
We have a new fetal surgery program that is starting and the surgeons are requesting a few medications to administer to the fetus. The request is to mix 3-4 medications into 1 syringe in order to reduce having to access the fetus and interupting the sterile field. We are reaching out to some other institutions that supposedly do this, but curious to see if anyone else does fetal surgeries and how they supply the medications for the fetus.
