Our system Alerts Optimization Team is lead by Medication Safety in partnership with Clinical Informatics/ Willow (Epic). We are re-evaluating our committee structure and scope, and plan to develop a charter. If you have a charter in place for your alerts/Clinical Decision Support team, would you mind sharing your charter with me at lrfrantz@novanthealth.org? MAR warning messages - Hazardous Medicines - Occupational Exposure

Dear all,

We're interested in what advice other health services provide on their EMR/MAR in relation to the occupational risk posed by cytotoxic and other hazardous medicines.

- Are MAR notes or labels used?
- How is it presented?
- Is PPE advice provided?
- Do you link to external resources for this information?
- How is this information maintained (e.g., for new medicines)?

Displaying weight in LBS - pros/cons?

We finally have gotten traction on locking our Meditech weight entry to kg only. The offer from IT is to lock out the pounds and ounces fields from data entry, but still allow a calculation and display on the screen.

I can see the value in having the lbs displayed instead of using conversion charts, but I also see how this can still lead to error. Can anyone share their experience with such a setup?

Drug Excursions-Temperature/Humidity

Does anyone have a policy on Temperature and Humidity excursions for Medication Storage within Medication Rooms or Pharmacy. Looking for some good guidance on how long outside of the temperature range before actions are taken (moving items to a new storage space). I have found information concerning manufacturers expiration dating where during their testing they don't even create an reportable incident unless the temperature is greater than plus or minus the acceptable range sustained for a time greater than 24 hrs.

3% sodium chloride for Transport

For hospitals with pharmacy preparing Transport bags of medications and supplies, do you include 3% hypertonic saline in the Transport bag? If so, how do you safeguard it? Do you dispense it from pharmacy for the Transport team? We currently stock mannitol, but are recently having more issues with maintaining them crystal-free. Our Transport team is also requesting it based on TBI guidelines, though I am very hesitant to keep them in the Transport bag. Thank you! -Melody Sun, CHOC Children's

Outpatient Continuous Chemotherapy Compounding Procedures

Good Morning,

I am new to the home infusion patient population (transferring from inpatient practice). I am interested to know if anyone has a SOP regarding continuous infusion chemotherapy compounding practices. For example, for patients receiving a 46 hour continuous fluorouracil infusion, is there a best practice for incorporating intended overfill into the patients order (versus exact dose) to mitigate the need for a coordinated flush to ensure the patient receives the full dose.
