Defining primary versus secondary setup for infusion practice in SOP

We are venturing to define system standards of practice for which medication IV infusions are administered as primary versus secondary (small volume intermittent) setups.

Additionally, we are pursuing ISMP recommendations to reduce medication loss per 12/3/20 and 4/8/21 Acute Care newsletters.

Has any system navigated a SOP with this infusion setup content that they would be willing to share?

Also wondering if anyone has ventured to place primary or secondary setup in the EHR order or on the pharmacy label.

Tube feeds on the MAR

We are having issues with nurses not hanging D10 when tube feeds are stopped and patient received insulin. We have a request to document tube feeds on the MAR in order to facilitate clinical decision support.

If you have tube feeds on the MAR and use Epic, please provide information on your Epic build or provide a contact that I can use to help us with this.

Leap Frog Med Rec Audit

Hello! I was hoping to get some feedback from others who have successfully implemented the Med Rec Audit for Leapfrog.
Previously when this was not a reported metris, we had not implemented this due to multiple changes that were happening in our overall structure. Now that this is a reported metric, we are trying to figure out how to implement without adding an FTE or removing a current Pharmacy provided Service. This seems like it will require a significant amount of time, so we would love to hear other's experiences.

Recalls

Does anyone have a process for communicating information back to manufacturers or distributors regarding recalls? What info is included and what determines who gets a correspondence back? (ie severity of the recall, product identified etc)
any policy or procedure that outlines these processes would be greatly appreciated as well

Baxter Pump Pediatric Library

Hi dream team. Hope you are all well. We are currently working on improving our Pediatric Oncology library set up on the Baxter Spectrum IQ pump. We have some concerns regarding mg/m2 and/or mcg/m2 limits capturing potential overdoses. Is there anyone that currently uses the Baxter Spectrum IQ pump for Pediatrics? Would love to be able to see your library or pick your brain.
