Good morning everyone -

Reaching out to see if someone would share examples of communications or educations sent out to nursing staff regarding smartpump updates. We aim to do about monthly updates to the pumps and are trying to figure out the best formats and amount of information to share with staff regarding these updates.

Acetylcysteine in infusion pump

Hello all,

Would anyone be willing to share (especially if you have Alaris pumps) how you have Acetylcysteine infusion built in your smartpumps? Considering the concentrations and the dosing, it is quite a beast. Would love to know what you all have done!

Weekly Order "rewrite" in EHR

There has been some discussion in our institution about re-instituting a weekly order "rewrite" in the EHR similar to what was done back when we were on paper.

The thought that some have is that this will eliminate some of the errors we are seeing in relation to timing and meds "falling off the MAR."

I've been arguing against this practice as I think it will increase other errors and increase workload for all (physicians, nurses and pharmacists).

Can you provide me your pros/cons for this practice as well as any supporting literature?

Bolus from Bag - Safety Benefits of Barcode Scanning

Our BCMA compliance data revealed that we do not regularly scan the "Bolus from Bag" orders prior to administration (the order prompts the RN to scan the continuous infusion bag when attempting to document a given dose). Discussions with nursing have revealed that it is not always feasible to scan the bolus from bag prior to giving the dose (bolused through the smart pump as bolus from bag) due to the emergent needs of the patient.

Multiple Covid vaccine Preparations

What practices do your Covid vaccine sites have in place to separate Pfizer and Moderna vaccines from the preparation to administration stage? I did a brief search on here and saw some locations have created separate areas or use different color codes. Any other tips would be great!

ADC - first 5 letters of a drug name during product searches

We are about to take the upgrade from our ADC vendor in order to be able to require ISMP's recommended 5 letter minimum. If you have implemented this functionality in your organization, would you be able to share your lessons learned? Please message me if you are willing to share.

Oxytocin - Pump Programming

We currently have our pump library setup with an oxytocin induction entry that is for 1-20 milliunits/min and then we have a post-partum entry in which is run at 350 mL/hr. We have lots of 'events' that show up on our pump data involving oxytocin because they do not change over to the post-partum entry and the pump library hits the guardrail. Out smart pump team was wondering what other institutions have built out for their oxytocin and if/how they differentiate between induction and post-partum infusions. Thanks!
