Does anyone have neonatologists that order 0.2% Sodium Chloride? If so, do you recommend alternate therapy (i.e. D5/0.2NaCl) or what is your process to safely prepare in the pharmacy?

Nitroglycerin boluses requested for Cathlab have been drawn up in plastic syringes with 24 hr expiration and placed in amber bag upon delivery. Is anyone else familiar with this practice? typically boluses are drawn straight from untampered original bottle. Thanks for any insights
Does your order entry system automatically calculate IBW for patients who are under 5 ft tall?
If so, what formula does your system use?
Do you have one formula for patients 5ft tall or greater and a different one for patients less than 5 ft tall?
Do any of you use the CDC Growth charts to calculate IBW?
Does anybody use barcode scanning for medication administration in ambulatory settings, such as outpatient infusion centers? If so, what EHR platform do you use and when did you implement this technology?
Hello! How does your institution handle items like EZ-Cat barium for CT scans? Does pharmacy distribute? Does radiology solely handle it? Is there a prescription written? Thank you!

I'm wondering if others who use the 5gm lidocaine 4% cream have had patient complaints about the product 'burning' when applied? Our Wound Center recently switched to the 5gm lidocaine 4% cream as their smaller product; they also carry 30gm Curacaine 4%. The patients have started requesting not to have the 'small tube' because of the burning.
I'm wondering if this is an isolated to our Wound Center or wide spread. The Wound Center is working with the distributor to return.
