Umbilical catheter infusion fro line patency

I am wondering if there are best practice and/or standard concentrations used by neonatal units for UAC flush bags to maintain line patency. I am most familiar with hePARIN 0.5 units/mL and hePARIN 1 unit/mL but unsure if that is the standard of practice? Additionally, how do most facilities prepare this infusion? What hePARIN concentration product is used? Do you use the hePARIN IV flush syringe 500 units/5 mL syringe (100 units/mL 5 mL- classified as a device by FDA) or use hePARIN from another product(vial)? Do you add to prefilled NS or 0.45% saline bag or make exact volume?

Greetings!

One of our NICU pharmacists is asking pharmacy to draw up Albumin patient specific in IV room. Does anyone else have experience with drawing up albumin? If yes, can you please describe your process, and how do you clean up spills in IV room, since it’s a blood product.

Greetings,

How many sites have approved this medication for use?

We approved the above medication for use in the cath lab. Since approval, we have struggled with ensuring that this medication is used properly in terms of giving Brilinta while patient is getting the infusion or start a med after the infusion is stopped. How are other sites dealing with this? We are an Epic facility and medication orders on a navigator tab.

Sterile Water Removal

I'm new to the forum, so I apologize now for flooding it with new, and likely pre-discussed topics. This morning, I am curious of institutions that have had success in eliminating Sterile Water liter bags from their facilities. I have made a couple of attempts at this and have been blocked by a "legitimate clinical need" for this product. Specifically for making Sodium Bicarbonate drips. Has anyone been successful in this action and if so, how did you overcome the "legitimate need" issue?

IV Locks

Hello,

We have various IV locks built into our EMR. There is some debate of what the scheduled route nomenclature should be. Suggested nomenclature included "IV Dwell" and "IV lock".

What nomenclature does your EMR use?

We are in the process of increasing and promoting the correct use of carpuject holders as indicated by the ISMP IV Push guidelines. A question has come up about infection control techniques. How does your facility use carpuject holders? Each nurse has their own and cleans with alcohol? Autoclaves? or patient specific? other?
