We are about to begin our journey with Smart Pump Implementation and would like information from those of you who have completed this journey.

1) Who built/owns the drug libraries?
2) How often are your libraries updated?
3) Did you require additional FTE's for implementation or maintenance of drug libraries?
4) Who reviews data/reports from pumps and what is done with that information?
5) Are your pumps Integrated with your EMR? What issues have you had related to having or not having this?
6) Any other tips?

Hoping to learn how other organizations have built naming conventions for epidural products in their CPOE, pharmacy labeling and smart pump systems.

Do you list the narcotic component first and anesthetic component second OR the anesthetic component first and narcotic component second? (i.e. HYDROmorphone-bupivacaine or bupivicaine-HYDROmorphone)

Is this naming convention standard across all systems--CPOE, pharmacy labels and smart pumps all list same way, or are there differences?

Would anyone be able to share how IV insulin doses are prepared and administered in your OR and PACU areas? Usual doses would be 10-20 units IV x1.
How do the nurses draw up the doses? Do they use insulin syringes or TB 1mL syringes?
Is the dose transferred into another syringe, where the nurse further dilutes the dose prior to administration?

Does anyone use Prismasate/Sol for CRRT in their facilities? How often do you compound custom solutions and do you have ordersets to guide the physicians toward the standard concetrations rather than making custom solutions?

A colleague just noticed that the package insert for Intralipid 20% from Fresenius-Kabi says "Use a 1.2 micron filter with Intralipid 20%." The package insert from 2013 said "Filters should not be used with intravenous fat emulsion." I tried calling the company this morning, but they are closed for the holidays... Does anyone know why they changed their mind? We have never filtered lipids, as long as they are running by themselves. I'd like to know what Fresenius-Kabi is putting into their lipids that now warrants filtration.

Do any facilities require an independent double check for all oral chemo EXCEPT for hormonal antineoplastics (e.g. Megace, tamoxifen)? If so, what was your evidence/rationale? We are considering excluding hormonal antineoplastics from our IDC requirement. Our EHR is currently programmed to require a dual sign on ALL meds with an AHFS therapeutic class of "antineoplastic". I'd like to have evidence that giving too much of a hormonal antineoplastic will not harm the patient like other oral chemotherapies could...

We had a potential issue with a patient who had an aminoglycoside allergy entered and then had a reaction to flu vaccine when administered as an outpatient. At the time we were unaware that aminoglycosides are used in the manufacturing process for flu vaccine because our system database does not alert for that. We are trying to create a workaround for this alert and it seems to be taking more time than necessary

Has anybody had to deal with anything similar?

What alteplase concentration(s) does your institution use for catheter-directed thrombolysis procedures (e.g. 0.02 mg/mL, 10 mg/500 mL)?

If you’re using multiple concentrations, what safety measures have you put in place to minimize concentration mix-ups, specifically order entry errors and smart pump entry errors?
