We recently implemented Epic and have seen an increase in our drug/drug as well as therapeutic duplications. Does anyone have a protocol or policy they would be willing to share giving pharmacists the authority to discontinue certain medications without contacting the provider?

I would like to check for our colleges that work in hospitals with ambulatory care services, how many oral tablet Methotrexate strengths available in your formulary

Currently we have Methotrexate 2.5 mg only, this was the case for a very long time.

Recently, we got a request to add methotrexate 10 mg tab to our hospital formulary, to make it more convenient for patients with high doses not to take high number of tablets.

I am really worried about the possibility of medication error and the consequence of error specially with this type of medication
