Oral methotrexate available strenghts

Dear all;
I would like to check for our colleges that work in hospitals with ambulatory care services, how many oral tablet Methotrexate strengths available in your formulary

Currently we have Methotrexate 2.5 mg only, this was the case for a very long time.

Recently, we got a request to add methotrexate 10 mg tab to our hospital formulary, to make it more convenient for patients with high doses not to take high number of tablets.

I am really worried about the possibility of medication error and the consequence of error specially with this type of medication

Hello,

This question has been posed in the past, but I would like to survey the group again:

For patients receving parenteral opioids, do you have specific policies around the nature AND frequency of monitoring (e.g., oxygen saturation, respiratory rate, pulse, blood pressure, or other forms of monitoring)?

If yes, could you please share?

I would be particularly interested to hear from sites who care for pediatric patients.

Labeling multi-dose meds for home use

Is anyone labeling their multidose meds with an outpatient label at the time of initial inpatient dispensing? For example, an Advair inhaler would have a label indicating name/address/phone number of the hospital pharmacy, "Take as directed on Discharge Medication Instructions", etc... I like this idea because it improves the transition to outpatient care and reduces pharmaceutical waste.

How are you handling ISMP targeted best practice #2: Patient Education by a pharmacist for all weekly oral methotrexate discharge orders? We are an Epic hospital.
How is the pharmacist notified?
Where is the patient education housed?

Infliximab

Has anyone been seeing increased reactions to infliximab? We have had several reactions in the last few weeks (most minor) but it is noticeable enough that we are looking into lot #'s, etc.

Mg/Mcg mixups

I wanted to query the group to see if there were any best practices around preventing mg and mcg mixups. Specifically, if there are practices that help prevent errors associated with medications dispensed as a mg dosage forms (i.e. norepinephrine 8mg/250ml) but dosed in mcg/kg/min or mcg/min. Thank you,
