Chemical restraints monitoring

Good Afternoon Members- Wondering how you monitor the use of chemical restraints at your institution. Which meds are included? Is it retrospective or prospective review? How you capture this data from CPOE? Is there a workgroup to review patients on these meds? Who is responsible for audit?
