Double checks in cath lab & procedural areas

Hi: wondering if anybody has figured out how to conduct an effective double check in procedural areas like the cath lab. We have been struggling with the logistics of doing this: there is only 1 nurse present & available and the physician is focused on the case. Yet we recognize it is important for high risk drugs and that there are likely errors occurring that are unrecognized. We also don't want to be setting up dual standards in our hospitals.

Any ideas/suggestions??
