Enoxaparin Dispensing -- Small Doses

For enoxaparin preparation/dispensing for doses that are not commercially available in syringes:
1) Do you allow doses to be dispensed in insulin syringes?
2) Do you use a dilution for doses less than 10mg or less than 5mg?
3) What concentration do you make your dilution?

We currently allow using the 100 mg/mL product down to 5mg (then use a 20 mg/mL dilution) and do not use insulin syringes for any doses. However, we've anecdotally noticed that after switching away from using insulin syringes for doses 5-10 mg, there was great variability in the enoxaparin anti-Xa's.
