Hospital Pharmacy Labels - Checklist

I am working to develop a checklist of sorts to evaluate the many templates of label types available within EPIC.
Does anyone have a checklist or any ideas to consider when developing this for a proactive evaluation tool?

For example: 2 patient identifiers - which 2?

Intravenous admixture labels:
1. Name and location of the patient
2. Generic medication name
3. Name and amount of the base solution
4. Dose, strength, and/or concentration of active medication in the final product
5. Date prepared
6. Expiration / Beyond use date (and time if the expiration occurs in less than 24
hours)
7. Special storage requirements and any applicable accessory labels
8. Directions for use
9. The name or initials of the person preparing the admixture and the pharmacist
performing the final check
