Methocarbamol associated phlebitis

One of my facilities frequently administers methocarbamol slow IV. Recently, they acquired the generic product from Somerset Therapeutics and have had a few instances of phlebitis, even when diluted and given very slowly. I'm reaching out to see if this is an isolated issue or if other facilities administering slow IV push have noted an uptick in phlebitis. Of note, they have not had this issue in the past with the brand name product.
