Near Miss Documentation Incentive

Does anyone have a successful way of encouraging documentation of near miss events in an ambulatory setting? Our Specialty Pharmacy has a quality initiative to improve near miss reporting with discussions currently centered around tying reports to the team member annual evaluation. I want to discourage this so looking for alternative solutions to recommend. Thank you!
