Notifying patients of automatic substitution of their prescription

For those of you with outpatient pharmacies within your system, how do you notify patients that a medication they were previously taking has been automatically substituted to another medication? We are looking to prevent patients from taking both the old medication and the new substituted medication.

What is your process for patients who come to the pharmacy to pick up their med?

What is your process for patients who have their medications delivered to them?
