Small Pediatric Dosing

I am wondering how hospitals with pediatric units manage orders for doses that are < 0.2mL (or some other small value)?

Does the pharmacy draw all of these doses and send to the unit?
What is your cut-off volume for pharmacy preparation?
What are your EHR dosing rules to accomplish this?

Our pediatric units have the syringes to draw up the smaller doses but are drawing up incorrect amounts (i.e. 0.6mL for 0.06mL dose). We still have certain medication vials in Pyxis and would like to keep them available but have the smaller doses default to the pharmacy for preparation.

I would love to hear what others are doing. Thank you for your time.
