ADC 5 Letter/Character Requirement

Hi All,

Looking to connect with an organization that has implemented the ISMP best practice around requiring 5 Letters/Characters during ADC drug searches via override. In reading past MSOS threads, most were still in the planning phase or stepped back from 5 characters to 4.

Please share in this thread any learnings or feel free to email me directly! I am also happy to set up a quick chat if that's easier.

Pill cutter use at ADC

Wondering what other organizations do to support real time wasting when it comes to tablet cutting at the ADC. Our compliance department feels the pill cutters should be single patient use, which makes sense if the enter a patient room, but when used in a clean med room I'm not so sure? Our nurses don't want to clean the cutter between uses due to risk of cut with the blade. What does your organization do?

ADC Returned Meds by a Nurse

What is the return to pharmacy/ADC process look like at your institution? Do you have internal and/or external return bins for your ADCs whereby users/nurses return any unused medications to the return bin? Do you allow/expect returns to be done back to the ADC pockets by nursing? Do you have the same return process for controlled substances and non controlled substances?

Pyxis Not Displaying Suffix (Jr./Sr.) for Nursing

Hello,

We recently had a near miss event at our institution where 2 patients with the same name (one Jr, one Sr.) almost received medications intended for the other patient.

Upon review, one of the gaps we identified was that the patient suffix does not display for nurses on Pyxis ES console. According to our BD Carefusion representative, the patient suffix information is transmitted over to Pyxis, but is information only and cannot be displayed for nursing at the console.

Pharmacist check ADC replenishment stock

Does your organization have a pharmacist check all non-patient specific ADC replenishments (pyxis, omnicell...etc)?

If not, do you use a tech validation or tech-check-tech program? and\or do you rely on barcode technology on dispense and fill?

We have always had a pharmacist check pyxis replenishment stock and have not (re)-evaluated in a number of years since incorporating barcode validation on dispense, and on pyxis fill.

appreciate others perspective and any value added accounts of having a pharmacist check pyxis stock.

Partial bottles of anesthesia gas

Current practice in our OR is anesthesia fills their machines with the gas and then the bottle is returned to the ADC to ensure these are locked up. The problem, however, is the bottles being returned are partial (open) bottles. Thinking about maintaining integrity, would there be any concerns with these partial bottles remaining in the ADCs? Outside of keeping these locked up, the only other option would be to top off the reservoir and then waste the remaining amount. We want to ensure we can speak to the integrity if anything is mentioned during a survey.
